Part 1: Problem Identification and Description
Sexually transmitted diseases (STDs) and unwanted pregnancies are major problems in the society. They affect not only adults, but adolescents as well (Wagenlehner et al., 2016). An unwanted pregnancy refers to a pregnancy which is not wanted, for instance it occurred when a child was not desired. It also arises when a woman became pregnant earlier than desired (Herd et al., 2016). The contraction of STDs and unwanted pregnancies amongst grownups and teenagers, which is the identified problem, often arises as a result of engaging in unprotected sexual activity. The purpose of this paper is to describe the problem and the significance of the evidence-based project and how to create awareness and safety promotion.
Clinical Setting
The clinical setting comprises primary care offices for men and women. It also includes obstetrician gynecologist (OBGYN) clinics that provide services related to women’s health. Furthermore, it encompasses college health clinics that serve young adults, Planned Parenthood clinics, schools attended by young adolescent boys and girls, urgent cares, and emergency rooms (ERs). Therefore, the clinical setting consists of various facilities and institutions that can cater to both males and females.
Service Area for the Clinic and Population Groups
The service area for the clinic include offering services such as vaccinations, routine health screenings, and basic laboratory testing. It also provides treatment for STIs as well as Planned Parenthood, urgent care, and emergency care services. Additionally, the clinic provides obstetrics and gynecologic care for female patients of all ages. The population groups who attend the clinic services consists of adolescents and adults. Both male and female patients visit the clinic on a regular basis. Although people with an extensive range of problems visit the clinic, there are a few specific kinds of problems that are common among most of the patients. One of them is related to unwanted pregnancy. A large number of women and adolescent girls often visit the clinic to consider their options for unwanted pregnancy, including abortion. Depending on the length of time she has been pregnant, the pregnancy can be terminated through a medical procedure or by simply taking medication. The other problem commonly seen in the clinic is adult men and women as well as adolescents who present with different kinds of STDs, particularly HPV, herpes, and chlamydia, and occasionally HIV, syphilis and gonorrhea.
Problem Statement and Target Population
Over the past several years, increasingly more adults and adolescents have presented to the clinic with the problem of unwanted pregnancies and contraction of STDs. This shows that the problem is, in fact, growing hence the need for an appropriate evidence-based intervention to address it. The target population is large enough. This is because it comprises both adolescents and adults of both genders who are engaging in unprotected sexual activities thus exposing themselves to the problem of STDs and unwanted pregnancies. As such, the problem could be analyzed from many conceptual and theoretical viewpoints, and appropriate plans could be made for the purpose of addressing the problem from different perspectives. Besides, the identified problem is of enough interest to me to be brought into both male and female genders. Assessing all its dimensions, planning in detail how to address it, and designing a detailed intervention and evaluation plan to all populations is my focus.
Significance of the Evidence-Based Project and Knowledge Gap
The evidence-based project entails providing behavioral health interventions and education. It will be aimed at reducing the contraction of STDs and preventing unintended pregnancies in the patient population. This project is important to me as it will allow me to make an impact in the society with regard to the prevention of unwanted pregnancies and STDs. Thanks to this project, the participants would be able to learn more about reproductive issues and they may consistently utilize contraception or STI protection.
At the moment, there is knowledge gap with regard to the efficacy of behavioral health interventions and education in addressing the problem among both adolescents and adults. There are a few clinical observations that point to this gap. Firstly is the fact that most patients who visit the clinic for treatment of STDs, particularly adolescent ones, seem to be less educated as regards the effects of such diseases on them and how they can protect themselves for instance by receiving human papilloma virus (HPV) vaccination. Secondly, many female patients appear to have little knowledge about contraception and how to prevent unwanted pregnancies.
Several studies have been carried out in the area that delineate the gap in knowledge. In their study, Barandouzi and Cong (2019) sought to evaluate the knowledge of STIs amongst students studying in college. A total of 345 undergraduate students aged 18-22 years from a college in America took part. As per their findings, these young adults lacked knowledge of STDs. Besides, respondents who attended courses on sexual education attained higher knowledge scores on STDs in comparison to those who did not attend (Barandouzi & Cong, 2019). In another study involving 350 engineering college students aged 16-24 years, Subbarao and Akhilesh (2017) found out that nearly all of them were aware of HIV as an STD although less than half of them were aware of other sexually transmitted infections. This study was done in Asia. The researchers also learned that educating students about safe sexual practices and sexual health helps in controlling and preventing STDs (Subbarao & Akhilesh, 2017).
In a research carried out in India, Davis, Sarasveni and Krishnan (2020) learned that college students had moderate knowledge about emergency contraception for preventing unwanted pregnancy. They also reported that reproductive health education improves awareness and removes misconception as regards emergency contraception. A total of 758 participants aged 18-28 years took part (Davis, Sarasveni & Krishnan, 2020). Overall, most of the studies on the topic have been carried out outside the United States. Also, they have mostly included adults. There is therefore gap in our knowledge about the effectiveness of behavioral health interventions and education in the context of the United States, as well as within the adolescent population. Therefore, by carrying out the project in America and involving both adolescents and adults, the gap in knowledge will be addressed.
Conclusion
In summary, the identified problem is STDs and unwanted pregnancies amongst adults and adolescents. The lack of knowledge and information in regards to long-term effects of STDs, prevention, and safe sexual practices in males and female young adults is unjustifiable. The clinical setting mainly consists of primary care offices for men and boys, OBGYN clinics for women, as well as Planned Parenthood and college health clinics. The kinds of problems seen in the clinic mainly comprise individuals seeking treating for STDs or women and girls who want abortion. The evidence-based project entails providing behavioral health interventions and education to the target population that consists of adolescents and adults. The project is significant, as it will help to decrease the contraction of STDs and incidences of unwanted pregnancy in the patient population.
